Putting a call on hold and hanging up the receiver disconnects the call

This is a known issue. Please upgrade the phone firmware to the following releases which should fix the problem.
SIP-D28P Phones: Firmware Release 2.41.0.60
SIP-D26P Phones: Firmware Release 6.41.0.10
